This section is to be used as a resource for a Queen's University student-athlete. Forms & Policies are available in various formats including HTML, PDF and MS Word. Forms on this site may be viewed, filled-in on screen, printed and sent by regular mail.

This forms repository/library contains only the most current forms. To avoid using outdated versions, Athletics & Recreation recommends that you do not download these files to your hard drive.
